间充质干细胞(MSCs)因其多向分化潜能、自我更新能力以及促血管生成、免疫调节和抗炎等生物学特性,在糖尿病足溃疡的治疗中展现出广阔应用前景。大量临床研究和系统评价表明,MSCs移植总体安全,严重不良事件发生率低,主要副作用轻微且可逆。MSCs可通过改善局部缺血状态、促进血管新生和加速创面愈合而发挥治疗作用,部分随机对照研究和荟萃分析显示其在溃疡愈合率、肢体灌注指标及生活质量改善方面优于常规治疗。不同组织来源的MSCs在增殖能力、免疫调节效应及临床疗效方面各具优势。然而,MSCs的临床转化仍面临细胞质量差异、制备流程缺乏标准化、规模化生产受限以及长期安全性和疗效尚不明确等挑战,仍需开展更大样本量、设计更严谨的临床研究,以进一步明确其疗效评价体系和最佳应用策略,推动MSCs在糖尿病足溃疡中的规范化和可持续临床应用。

Mesenchymal stem cells (MSCs) have shown great promise in the treatment of diabetic foot ulcers due to their multipotent differentiation potential, self-renewal capacity, and biological characteristics such as pro-angiogenesis, immunomodulation, and anti-inflammation. Numerous clinical studies and systematic reviews have demonstrated that MSC transplantation is generally safe, with a low incidence of serious adverse events and mild, reversible major side effects. MSCs can exert therapeutic effects by improving local ischemia, promoting angiogenesis, and accelerating wound healing. Some randomized controlled trials and meta-analyses have shown that they are superior to conventional treatments in terms of ulcer healing rate, limb perfusion indicators, and quality of life improvement. MSCs from different tissue sources each have their own advantages in terms of proliferation capacity, immunomodulatory effects, and clinical efficacy. However, the clinical translation of MSCs still faces challenges such as variations in cell quality, lack of standardization in preparation procedures, limitations in large-scale production, and unclear long-term safety and efficacy. Larger sample sizes and more rigorously designed clinical studies are still needed to further clarify the efficacy evaluation system and optimal application strategies, and promote the standardized and sustainable clinical application of MSCs in diabetic foot ulcers.
